סרט ההמשך לתולעת האנושית (החלק הראשון). מרטין שגר עם אימו בפרויקט דיור מוזנח מתבודד בשל בעיות נפשיות. הוא עובד כמאבטח במשמרת הליילה בחניון תת-קרקעי עגום. כדי לשפר את מצב רוחו הקודר,מרטין שוקע בעולם הפנטזיה של סרט האימה "התולעת האנושית (חלק הראשון)" . סגידתו למיומניות כירוגיות קפדניות של ד"ר הייטר המחונן המומחה במערכות הקיבה והמעיים האנושיות נותן השראה למרטין להגיע אל הבלתי-ייאמן.
